Checkbox / No answer block for questionnaires

I'd love to have my clients being able to answer question via bullet checkbox and being able to have a block of customisable text to be able to separate the type of answers.

Also would love to be able to have side by side blocks for answers if block-like design is in the works. being able to drag and drop pre-made questions would be so cool to personalise questionnaires for each client when needed.
