Slightly more editable booking form

The new booking form is like 95% of the way there for me but I’m still not quite able to use it. My session options have specific names so the types of sessions being required but not customizable makes the form unusable for me.

Also, if we could get the custom questions option to have not just a text box answer but also options for dropdowns/check boxes/radio buttons that would be extremely helpful!
